My Records Request Position
I have requested all records pertaining to me over the entire course of my relationship with Swedish, not just a limited subset of encounters.
In my experience, Swedish continues withholding parts of my record even after multiple HIPAA requests over about three years.

- Records that could clarify who was present during the angiogram
- Records that could clarify what disclosures were documented
Why These Records Matter
These records matter because they may help answer basic questions about what happened, what was disclosed, who was present, and what information was later removed or omitted.
